Physical therapy claims are denied because of little or no advancement more than any other outpatient service. Payers look for measurable functional improvement to be presented at regular intervals, and if there is no functional improvement or a new goals/treatment plan, then continued visits begin to appear on paper as medically unnecessary. The recording of that treatment trail is something that needs to be carefully monitored with PT billing services, because it will be the same language over and over for each visit, which will prompt a medical necessity review by the payer.
